The same thing has gone wrong in three places and nobody told you what it was.
Different team, different manager, different building, and by month eleven the same shape returns. The working relationship that cools without an argument. The task handed to somebody else with a reason attached. The meeting where you were right and it went nowhere anyway. Each time there was an explanation, and each one was about that place. Three places is not three explanations. Something travelled with you, and nobody has watched enough of your working life to name it.
The bill arrives in pieces small enough to absorb. A reference that was warm rather than insistent. The responsibility quietly reallocated after a handover you thought had gone well. The colleague who stopped bringing you things early and started bringing them finished. None of it was said out loud. All of it was settled by people who had watched you work and reached a conclusion nobody would put in writing, and you are the only person in the building operating without it.

The instruction in this domain is to collect reports: the appraisal, the anonymous round of questions, the trusted colleague taken for a drink, the profile assessment that returns four letters or a colour. Every one gathers what people positioned in front of you could see and would say, in a setting where saying the whole thing had a cost. When it comes back flattering and useless you are told to ask better questions, ask more people, ask more often. So the method prescribes more of itself, and eight years later you still cannot name the thing.
It also reports incidents. An incident is a single occasion with a date on it, and the conduct that has followed you through three places is not an occasion. It is an angle. It sits behind you, it has been working in every post you have held, and you have never once been positioned to watch it, because on each occasion it appeared you were the one busy producing it. Your team can describe the sharp reply you sent in March. Nobody has watched enough ordinary days to describe how you close a discussion, and that is the one costing you.
A chart read is not a better witness. It is not a witness at all, and that is the difference. Every account you have been given came from somebody with a seat in the room, a working relationship to protect, and a version of you formed on the days they were there. A whole-life read comes from a source that was never in any of those rooms and holds no position in the next one. It describes the disposition rather than the episodes. It is written by something with no stake in whether you enjoy it and no reason to protect the working relationship afterwards.

None of these is answerable from inside your own seat, because each one concerns conduct other people have observed for years and priced in without mentioning it. What each one costs you moves with the calendar, and the report dates the movement.
Why the promotion is what hides it
The crossover never gets flagged, and the reason is structural: on the day the strength turns into the complaint, you are still doing the exact thing you were promoted for. There is no moment for anyone to point at. The same conduct simply arrives at a grade where thorough reads as slow and decisive reads as will not be told, and the people saying so are not the people who praised it.
The last time somebody corrected you
Name the date. For most competent people it sits further back than they expect, and the reason is not that the errors stopped arriving. It is what you did in the ninety seconds after the last one: supplied the context, produced the reasoning, corrected the correction. All of it reasonable, and all of it read across the room as a price. Corrections do not stop because you improved. They stop being offered.
Why work reaches you late, or already finished
Every post puts you in front of a call you do not have enough to make. Some hold out for a clarity that is not coming. Some commit early and then defend the commitment as though defending it were the decision. Your colleagues have known which one you are for years and route around it: the shift swap put to you too late to refuse, or the job brought half finished so you cannot take it over.
The conversation you move by a week
One class of exchange gets deferred, and it is the same class every time: the pay conversation, the one about somebody's performance, telling the family or the client that the date has moved. That is not a diary problem. Everybody downstream of you has already built a workaround for it, which is why nothing visibly breaks, and why the eventual cost lands on somebody who cannot say where it came from.
The failing nobody will name because naming it sounds ungrateful
Some of what you keep missing is not an error repeated. It is something you do too much of: redoing work you were handed, absorbing the task nobody assigned, holding a standard the place never asked for and resenting people for the shortfall. From your seat that reads as diligence. From every other seat it is priced as unreliable throughput, and nobody says so, because saying it sounds ungrateful.
Whether this is the run or the whole shape
A habit that has been working in every place you have worked and a habit belonging to the chapter you occupy now are indistinguishable from inside this month. One is worth twenty years of correction. The other resolves when the chapter turns, and grinding at it with a coach in the meantime is effort spent on weather. That distinction sits at the scale of a whole life, not a quarter.

The document closes in three days. The conduct does not.
The read answers it in writing: what you are disposed to miss in a room, what happens to your judgement when you are contradicted, and the errors your chart expects of you. It is the first written account of how you work that no employer authored. Your chart resolved once, the sitting where you described your own conduct, the warnings set beside twelve dated months.
What you said in the sitting stays retrievable, so your account of the clash with your line manager is what the astrologer reads the next one against, two posts later. A reorganisation lands and you must make the case for your job: ask whether the run ahead favours pressing it in the room or writing to one person. On whether a grievance is worth raising it does not advise. That is your union representative.
A period in your chart changes
Your conduct does not change at a handover. What it costs does. A run that forgave you for deciding alone gives way to one where the same call reads as freezing people out. You get what is ending, what is arriving, and the one adjustment worth making, often as small as routing the decision through a colleague first.
A date your report named is approaching
The read dated the runs where the same behaviour costs most. Ahead of one it names the error to keep off the table that month: the reply sent inside the hour, or the correction issued in front of everybody when it could have waited until after.
You raised a decision and went quiet
You said you would speak to the person whose work you have quietly been redoing, and then two weeks went by. Silence settles it. The conversation not had is the arrangement kept, and they go on believing the work was fine. It comes back once for the answer, and holds whichever version you actually chose.

Whether the thing that followed you through three places follows you into a fourth is settled by you, in ordinary exchanges this document has no vote in.
Your manager is not in this document. Neither is the reorganisation decided two levels above you, nor whether the colleague you keep clashing with is being told something different about you in a room you are not in. What it sets out is the conduct you are disposed towards and the runs across the twelve months where it is most exposed. What it costs you next year gets settled in rooms this read has no access to.
And it is one instrument among several. A manager can tell you what happened last quarter. A coach works the behaviour week by week. Somebody twenty years into your job will tell you what the habit costs a grade up, and a recruiter will tell you how it reads from outside. An employment lawyer is who you want when a process has gone wrong. None is scoped to describe what you have been doing the whole way through.
